Grand Avenue
Grand Avenue is a 5-bed home located in the South Park neighborhood of Santa Rosa. It provides a permanent living environment for people with behavioral health difficulties. The program provides affordable rent and includes support and minimal case management services.
Staff work with residents to manage household issues by problem solving as a group, to conduct house meetings, and to provide a forum for educational groups including independent living skills, medication education and communication.
Eligibility
Potential residents of this HUD-funded program must:
- Be an individual with a disability (we serve individuals with severe and persistent mental health conditions)
- Have income classified as extremely low/low
- Meet the HUD definition of chronically homeless
