Independent Transitional Housing with Case Management
The Bridges program is a 10-bed independent transitional program supported with case management services, in Santa Rosa. This goal of this program, in partnership with Sonoma County Behavioral Health, is to support residents as they plan for and make decisions about what they want for their future.
Residents begin with building relationships within the Bridges community, while compassionate staff support them in developing important community connections as they begin volunteering, working, or going back to school. Residents meet with on-site staff daily to set goals, review plans, and participate in case management services. Goals include medical and mental health, money management, housing opportunities, job and education, and accessing community resources such as local food banks and public transportation.
Residents also meet for required program groups during the week for further support from each other and with staff. They are actively supported and encouraged to get involved in programs and groups such as AA, 12 Step groups, Seeking Safety, NA, and/or dual recovery support groups. The typical stay within the program is around 9 months.

Potential residents must be:
- 18-59 years old
- Clients with Sonoma County Behavioral Health
- Referred to CSN through the SCBH Linkage committee
- Funded
