Case Mangement is a collaborative process of assessment, planning, facilitation and advocacy in order to meet an individual's mental health care needs and promote independence.

Case managment focuses on delivering personalized services to clients to improve their care, and involves the following steps:

  1. Screening to find appropriate participants
  2. Evaluation of each client to determine areas of need
  3. Developing a care plan to best serve each client
  4. On-going evaluation of overall program and its effectiveness

The goal of case management is for clients to take more control of their lives.  Case managment is to assist people to do what they can do for themselves.  Participants have strenghts and resources as well as problems and challenges.  Case Mamgement attempts to enable participants to use their strenghts and resources to overcome their problems and challenges and live a more fulfillin life.  Participants are responsible for the outcome.  The Case Manager is responsible for the process.
